English version

Conclusion. Single-use surgical instruments are used once, so reprocessing tolerance and long-term durability need no consideration, while reliability during that one use must be high, since instrument failure during surgery has direct consequences. That gives a different set of priorities from reusable instruments. Not required: reprocessing tolerance, long-term durability and cleaning accessibility. Required in depth: functional reliability during a single use, condition on opening the pack, and sterility assurance. Single-use instruments also typically use different materials from reusable ones, favouring plastics and lower-cost alloys, so material-related items adjust accordingly.

Types and items. Cutting instruments such as blades and scissors require sharpness, cutting performance and strength. Grasping instruments such as forceps require grip force, jaw engagement and opening and closing. Penetrating instruments such as trocars and needles require penetration force, strength and sharpness. Stapling instruments require staple formation, firing force and reliability. Retractors require strength and locking. Suction and irrigation devices require flow, freedom from blockage and connection integrity. And electrosurgical accessories require insulation, conduction and temperature rise.

Examining functional performance. Cutting performance covers sharpness, the force required and the quality of the cut. Grasping performance covers grip force, jaw engagement and freedom from slipping. Operating force covers opening, closing and firing, since excessive force impairs handling. Smoothness covers freedom from binding and noise. Repeated operation matters because an instrument may be used dozens of times within one procedure. And consistency covers variation between batches and between units. Reliability over repeated operations deserves attention: single-use does not mean used once and discarded, since one instrument may operate dozens of times in a procedure, and every one of those operations must be reliable.

Strength and failure. Structural strength covers the forces of surgical use. Fracture risk matters because fragments left in the body are a serious event. Deformation reduces function. Connection strength covers joints between components, particularly metal to plastic. And the failure mode should be perceptible rather than sudden fracture. Metal-to-plastic joints are the common weak point in single-use instruments, which typically combine plastic handles with metal working ends, and joint strength warrants specific verification.

Electrosurgical accessories. Insulation performance covers integrity and dielectric strength, since insulation failure causes unintended tissue burns. Insulation durability covers retention through a single use. Conduction covers the working end's conductivity and contact resistance. Temperature rise covers heating in use, particularly at the handle. And compatibility with the generator covers interfaces and parameter matching. Pinholes in insulation are the risk point for electrosurgical instruments: a tiny defect can burn tissue remote from the surgical site during use, where the operator cannot see it.

Materials and sterility. Material composition covers metals and plastics. Biocompatibility follows contact nature and duration; although intraoperative contact is brief, it involves tissue or blood. Processing residues cover cutting fluids, release agents and cleaning agents. Particulates cover what is present on instrument surfaces. Sterility requires sterilisation validation and sterility testing. And packaging must provide a sterile barrier, permit aseptic presentation and protect sharp features. Protecting sharp features serves two purposes: preventing them piercing the pack and causing contamination, and preventing injury during presentation.

Differences from reusable instruments. Materials can be lower-cost provided they meet single-use requirements. Durability need not be long-term while reliability within one use is essential. Reprocessing does not apply, though the instrument must be clearly marked as not for reuse. Cost is low per unit while volumes are large. And consistency requirements rise, since mass production makes batch consistency more important. The single-use marking matters: single-use instruments are in practice sometimes reused, and their design does not support it, since materials may not tolerate disinfection and the construction may not permit thorough cleaning.

Batch consistency. Volumes are large and consistency is correspondingly important. Key dimensional consistency affects fit and function. Functional performance consistency covers batch variation in sharpness and grip force. Material consistency covers the effect of raw material batches. Sterilisation consistency covers effectiveness across batches. And sampling plans follow batch size and risk. Compare key functional parameters across batches, since users of single-use instruments cannot judge from the feel of a used instrument and depend entirely on manufactured quality.

How we handle it. For single-use surgical instruments we suggest including reliability over repeated operation in functional testing, measuring not only first-use performance but whether performance holds after repeated use. For electrosurgical accessories, insulation integrity is an item we always test, since burns caused by insulation defects are both concealed and serious. We verify metal-to-plastic joint strength specifically, since that is the characteristic weak point of single-use instruments.
