English version
Conclusion. Medical device packaging usually has several layers: a primary pack in contact with the product, a secondary pack around it, and a shipper. Not every layer serves the same function. The key question is which layer forms the sterile barrier, meaning the layer that keeps the product sterile until use; its integrity is central to sterility assurance, while the others provide protection, handling convenience and information. Once that is clear, the focus of validation is clear: the sterile barrier layer requires integrity validation and the others require protective performance validation. Applying the same validation to every layer wastes effort and may still miss the point.
Roles of the layers. The primary pack, as sterile barrier, maintains sterility, and validation focuses on integrity, microbial barrier and sealing. The secondary pack protects, groups and carries information, and validation focuses on protective performance and marking. The shipper protects during transport and withstands stacking, and validation focuses on strength, cushioning and stacking. Cushioning components absorb shock, and validation focuses on cushioning performance and recovery. Note that the sterile barrier is not necessarily the innermost layer: in some products the innermost layer merely retains or separates, and the actual barrier lies further out. The test is which layer maintains sterility until opening.
Integrity test methods. Several categories apply to the sterile barrier layer. Physical methods, including bubble testing, vacuum decay, pressure decay and tracer gas, detect leaks quantitatively or semi-quantitatively. Dye penetration checks seals for channels, being direct while limited in applicability by material and seal form. Visual inspection checks for obvious damage, wrinkles and entrapped matter, being the most basic and still necessary. And microbial challenge using an aerosol verifies barrier performance directly, though the procedure is involved. Method selection must consider the packaging material and form, whether flexible, rigid or composite; the leak size detectable; whether the method is destructive; and whether it suits routine inspection. Detectable leak size is the key selection parameter: method sensitivities differ greatly, and the channel size through which microorganisms pass has a characteristic magnitude, so detection capability must match it or a no-leak-detected result proves nothing.
Barrier performance. Beyond the absence of channels, the material's own barrier capability requires verification. Microbial barrier performance covers the material's ability to exclude microorganisms. Permeability covers whether breathable packaging for ethylene oxide sterilisation is sufficiently permeable. Water vapour transmission matters for moisture-sensitive products. Oxygen transmission matters for oxidation-sensitive products. And light barrier matters for light-sensitive products. Permeability and microbial barrier must be balanced, since highly permeable materials tend to have poor barrier properties; purpose-made medical packaging materials reconcile the two through special construction, which ordinary materials generally cannot.
Interaction between layers. Layers are not independent. Outer layers protect inner ones, their cushioning determining the impact the inner layer experiences. Friction between layers during transport vibration can abrade the inner layer. Compression from stacking transmits through the outer layer to the inner. Water vapour can accumulate between layers where the outer layer is impermeable. And multilayer construction affects penetration of the sterilising agent. Abrasion of the inner layer by friction is a problem transport validation reveals: a thin inner layer rubbing against the outer layer or the product during prolonged vibration can develop pinholes, damage not readily visible and requiring integrity test methods to confirm.
Sampling and criteria. Sample size follows the severity of failure and statistical requirements, with sterile barrier work demanding more. Sampling must be representative across production batches, equipment and shifts. Criteria must be explicit as to what passes and what fails, set in advance. Borderline cases need a rule, covering what to do when a small leak is detected within the method's uncertainty. And samples must be reserved for destructive testing, since most integrity tests are destructive. Write criteria in directly judgeable form, such as leak rate below a stated value or dye not penetrating to the inner seal edge, rather than a general statement that the seal is intact.
Relationship to other validation. Packaging integrity does not stand alone. It relates to sterilisation validation, since sterilisation affects packaging and integrity should be assessed afterwards. It relates to transport validation, since post-transport integrity is the state that matters in use. It relates to stability studies, covering integrity retention across shelf life. And it relates to seal parameter validation, since the sealing process window underpins integrity. The suggested order is seal parameter window, then integrity after sterilisation, then after transport, then across shelf life, each step adding to the challenge of the last.
Production control. In-line or near-line inspection suits some methods. Sampling frequency follows risk and process stability. Visual inspection of every unit costs little and catches obvious problems. Trend analysis of inspection data reveals process drift early. And a rule must define batch handling when a failure occurs. Trend analysis is valuable for finding problems early: results all passing while leak rates climb steadily indicates drift, and adjusting then is far more proactive than waiting for a failure.
How we handle it. For packaging integrity work we confirm with the client which layer is the sterile barrier, since that determines the focus and method selection. Testing without settling it risks effort spent on secondary layers. On method selection we check detection capability against the leak size of concern, because a method of inadequate sensitivity produces a passing result that cannot support sterility assurance.
