English version

Conclusion. Infusion and syringe pumps deliver fluid into the body at a set rate, and their safety rests on two things: delivering accurately, and alarming when something goes wrong. Inaccurate delivery has direct consequences: too fast risks overdose, too slow fails to achieve the therapeutic effect, and for high-risk drugs the consequences of deviation can be serious. Alarms interrupt infusion and warn when something is abnormal: occlusion, air, infusion complete and low battery would otherwise go unnoticed for long periods. The test programme is long, but it centres on these two lines.

Organising the items. Electrical safety covers leakage current, dielectric strength, earthing, temperature rise and single fault conditions. Applied parts cover classification and isolation. Electromagnetic compatibility covers emissions and immunity. Flow accuracy covers correctness and stability across rates. The alarm system covers occlusion, air, completion, fault and battery. Occlusion items cover alarm thresholds, alarm time and pressure release. Free-flow protection covers what happens when the set leaves the pump. Battery testing covers endurance, changeover, charging and indication. Mechanical and environmental testing covers drop, vibration, temperature, humidity and ingress protection. And consumable compatibility covers sets and syringes. Free-flow protection is the item specific to these devices: when the set is removed or the door opened, unprotected fluid can flow freely under gravity and deliver a large dose rapidly.

Examining flow accuracy. Testing generally measures actual delivered volume by gravimetric or volumetric means and computes deviation from the setting. Verify across the whole settable range, low rates especially, since relative error is generally larger at low rates and clinical practice uses low rates for high-risk drugs. Examine short-term and long-term behaviour separately, since instantaneous deviation over a short interval and cumulative deviation over a long one are different things. Examine start-up characteristics, covering the transition from rest to steady delivery and the volume deviation during it. Examine the effect of back pressure, since the distal end of a real set carries pressure. Examine the effect of consumables, since set batches and brands affect flow. And examine temperature effects on fluid viscosity and pump mechanism. Low-rate accuracy is the focus: many devices behave well at moderate and high rates and deviate noticeably at low ones, while neonatal dosing and certain high-risk drugs use precisely those low rates. Start-up is also easily overlooked: some pumps lag or overshoot at the beginning, so that the dose delivered during that period differs from the setting.

Examining occlusion alarms. Examine the alarm threshold, meaning the pressure that triggers it and whether it is adjustable. Examine the alarm time from occlusion to alarm, which relates to flow rate, since pressure rises slowly at low rates and the alarm time may be long. Measure alarm time at several rates. Examine pressure release, since pressure accumulated in the set during occlusion can deliver a bolus when the occlusion clears and the device should release it. And examine alarm reliability, which should neither miss nor alarm spuriously. Alarm time at low rates concerns clinicians: infusing at a very low rate, pressure takes a long time to reach the threshold after occlusion, and during that period the patient receives nothing. Measure it and state it in the documentation.

Examining air detection. Examine detection capability, covering the bubble size detected and detection of accumulated air. Examine control of false alarms, meaning whether disturbances other than air, such as set position or fluid properties, trigger alarms. Examine behaviour after alarming, covering stopping infusion and warning. And examine compatibility with different sets. Accumulated air deserves attention: a single small bubble may not trigger an alarm while a succession of small bubbles accumulates into a significant volume.

Other safety functions. Dose limits cover single-dose and cumulative limits over a period. Setting error protection covers interception of incorrect input, such as a misplaced decimal point. Drug libraries, where fitted, require correct configuration and protection against tampering. Channel independence in multi-channel devices requires that one channel's fault does not affect others. And power failure protection requires stopping infusion and alarming, without resuming automatically on restoration. Setting error protection is a practical measure against medication error: a decimal point misplaced by one position changes the dose tenfold, and the device should recognise settings clearly outside the usual range and ask for confirmation.

Defining essential performance. For infusion devices this typically covers accuracy of delivery rate within the specified range; absence of unintended over-delivery; effectiveness of alarms; and effectiveness of the stop function. Write in the absence of unintended over-delivery explicitly: it is the higher-risk failure mode for these devices and belongs in the acceptance criteria for immunity testing.

Preparing for submission. Provide the matching consumables, submitting every size of set and syringe claimed as compatible. State the compatibility range, meaning which brands and sizes may be used. Provide parameter setting details covering ranges and methods. Provide alarm configuration details covering thresholds, priorities and adjustable ranges. Provide the essential performance definition and acceptance criteria. And prepare enough samples and consumables, since accuracy testing consumes a considerable quantity. State the consumable compatibility range clearly: sets from different brands differ in internal diameter and elasticity, flow accuracy can differ noticeably, and everything claimed as compatible should be verified.

How we handle it. For infusion devices we measure at several points in the low-rate region, since that is where accuracy problems concentrate and where clinical risk is higher. On occlusion alarms we suggest measuring alarm time at several rates and stating the low-rate alarm time explicitly, since that figure bears directly on clinical use, whereas a pressure threshold alone says nothing about the actual delay.
