English version
Conclusion. Urinary catheters and ostomy products share characteristics of prolonged body contact, with catheters indwelling for days to weeks and ostomy appliances adhering for days, and of managing bodily waste, where leakage causes contamination, infection and skin damage. Three concerns follow. Material safety under prolonged contact, since contact with mucosa or skin over long periods imposes demanding biocompatibility requirements. Leak prevention, since sealing or adhesion failure has direct consequences. And skin protection, since peristomal skin is adhered to repeatedly and contacts effluent, making skin problems a common complication.
Examining urinary catheters. Dimensions cover outer and inner diameter, length and balloon volume. Material properties cover softness, elasticity and tensile strength. Balloon performance covers inflated shape, pressure resistance, retention, leakage and completeness of deflation. Lubricity covers coating performance and durability. Flow covers drainage patency. Kink resistance covers whether bending occludes the lumen. Biocompatibility follows prolonged urethral mucosal contact. And surface characteristics affect bacterial adhesion and encrustation. Balloon retention is the critical item: leakage allows the catheter to be expelled, and rupture may leave fragments. Perform prolonged retention testing simulating the actual indwelling period. Encrustation is the long-term problem: minerals from urine deposit on the lumen wall and progressively obstruct it, with surface characteristics affecting the rate.
Examining ostomy products. Baseplate adhesion covers initial adhesion, durability and retention under moisture and perspiration. Baseplate materials cover skin contact and the protection they provide. Aperture cutting covers whether it can be cut to size and how the cut edge behaves. Pouch sealing covers the integrity of each seam. The outlet covers reliability of opening and closing, ease of emptying and sealing once closed. Filtration and venting cover gas release and odour control. Capacity covers pouch volume against actual need. And durability covers strength retention through the wear period. Adhesion retention under moisture is critical in real use: perspiration, bathing and effluent contact all affect adhesion, and verification should include those conditions rather than dry adhesion alone.
Materials and biocompatibility. Determining contact nature covers mucosa, intact or broken skin, and effluent. Contact duration covers catheter indwelling and appliance wear times. Item selection follows nature and duration. Irritation covers prolonged contact. Sensitisation covers the risk from adhesives and materials. And released substances require chemical characterisation. Adhesive sensitisation deserves attention in ostomy products, since adhesives contact skin continuously and users depend on them long-term, making sensitisation particularly troublesome.
Examining sealing and leak prevention. Static sealing covers normal conditions. Sealing under compression covers squeezing when the user moves, bends or sits. Sealing after emptying covers repeated opening and closing of the outlet. Prolonged sealing covers retention through the wear period. And seam strength covers the pouch when full. Sealing under compression is the real leakage scenario: bending or sitting compresses the pouch and raises internal pressure, and weak points may leak. Include pressurised conditions in verification.
Prolonged use considerations. Material ageing covers property change during wear. Adhesion decay covers the fall in adhesion over time. Cumulative skin effects cover repeated application and removal. Encrustation and obstruction cover the long-term catheter problem. And infection risk relates to surface characteristics, sealing and handling. Give explicit replacement interval recommendations based on material decline data rather than experience alone.
User experience considerations. Discretion covers pouch shape and profile. Odour control covers filtration and deodorising effectiveness. Noise covers sound during movement. Ease of handling covers single-handed operation and operation without clear sight. And comfort covers prolonged wear. None of these is a safety item while all affect the user's quality of life and adherence directly, and poor adherence brings health consequences.
How we handle it. For these products we suggest verifying sealing and adhesion under conditions simulating real use, covering moisture, compression and movement rather than static dry conditions alone. For catheters we suggest prolonged balloon retention testing simulating the actual indwelling period, verifying that slow leakage does not allow expulsion. On materials, biocompatibility and chemical characterisation for prolonged contact are the focus, with specimens in final condition including coatings and after sterilisation.
