English version

Conclusion. Dental implants are long-term implantable products, and their evaluation centres on two things: whether the structure can sustain masticatory loading over the long term, and whether the surface can achieve osseointegration. The first is mechanical performance, with fatigue testing at its core: mastication is high-frequency cyclic loading, and an implant must withstand millions of cycles across years or decades of service. The second is surface characteristics: surface topography, composition and roughness affect osseointegration, whose quality determines success or failure. The remaining items, covering materials, sterility, biocompatibility and packaging, are the general requirements for implantable products.

Organising the items. Mechanical performance covers static strength, fatigue and torque. Dimensions and fit cover dimensional accuracy and connection fit. Materials cover composition, microstructure and mechanical properties. Surface characteristics cover topography, roughness, composition and coatings. Biocompatibility follows long-term bone contact. Sterility covers sterilisation validation and sterility testing. Packaging covers sealing, integrity and shelf life. Marking covers product identification and traceability. And system components cover abutments, screws and instruments.

Designing fatigue testing. Loading should simulate the direction and position of masticatory load, normally applied at an angle to the implant axis to represent oblique forces. Load magnitude follows expected masticatory force, allowing for differences between anterior and posterior positions. Cycle count converts from expected service years and chewing frequency and is substantial. Loading frequency must not be excessive, to avoid heating affecting material properties. Environment should simulate body fluid, since corrosion combined with fatigue shortens life. Component assembly should be complete, testing implant with abutment and screw, since failures often occur at connections. And failure criteria cover fracture, loosening and excessive deformation. Testing in simulated body fluid is an important design choice: fatigue life in dry conditions exceeds that in body fluid, while implantation occurs in body fluid, so dry testing alone produces optimistic data. Testing complete assemblies matters equally: the implant body may be strong enough while the abutment connection or screw fails first, which component testing cannot reveal.

Static strength and torque. Static strength loads to failure, recording the load and location. Insertion torque covers the torque the implant can withstand during placement. Screw tightening torque covers reliability at the recommended value and behaviour when over-torqued. And connection stability covers micromovement at the connection under load. Micromovement deserves attention, since it can loosen screws, wear the connection and allow bacterial colonisation in micro-gaps.

Examining surface characteristics. Topography covers microscopic structure, examined microscopically. Roughness covers surface roughness parameters affecting osseointegration. Composition covers surface elemental composition, particularly elements introduced by surface treatment. Coatings, where present, require thickness, composition and bond strength. Cleanliness covers residues from machining and processing. And batch consistency covers the uniformity of surface treatment. Surface cleanliness is easily overlooked and matters: machining residues, blasting media residues and cleaning agent residues can all affect osseointegration and biocompatibility. Coating bond strength is critical for coated products, since delamination generates particles and impairs integration.

Material items. Composition must conform to specification. Microstructure covers the material's condition. Mechanical properties cover strength, elongation and hardness. Impurities and inclusions matter as potential fatigue origins. Corrosion behaviour covers performance in simulated body fluid. And metal ion release covers quantities released during long-term implantation. Corrosion behaviour and ion release relate to biocompatibility and are items requiring specific examination for long-term implants.

System components. An implant system comprises several parts. Abutments connect implant to restoration, requiring examination of both fit and their own strength. Screws are connecting components requiring torque and fatigue assessment. Healing caps are short-term components. Impression components do not implant but contact the oral cavity. And instruments including wrenches and drills must fit the product. Compatibility within the system must be explicit: which abutments fit which implants, since mixing can produce poor fit and early failure. State the compatibility relationships in the documentation.

Sterility and packaging. The sterilisation method must be compatible with the material and surface treatment. The effect of sterilisation on the surface requires verification, since some methods may alter surface characteristics. The sterile barrier requires integrity and shelf life verification. Double packaging facilitates aseptic presentation. And marking covers model, size, batch number, expiry and sterilisation method. The effect of sterilisation on the surface needs specific verification, since surface characteristics are the product's core performance and some methods may alter surface chemistry or introduce contamination.

How we handle it. For implant work we suggest conducting fatigue testing in simulated body fluid and using complete assemblies of implant, abutment and screw as specimens, both corresponding to the actual implanted state, since dry testing and component testing produce optimistic data. On surface characteristics we suggest comparative characterisation before and after sterilisation to confirm that the process has not altered key surface properties.
