English version
Conclusion. Wound dressings manage exudate and maintain an environment suited to healing. Their core performance centres on three things. Absorption and retention, meaning how much exudate can be absorbed and whether it stays absorbed under compression. Environment maintenance, meaning keeping the wound moist without maceration, since too dry delays healing and too wet macerates surrounding skin, which depends on the balance between absorption and moisture vapour transmission. And behaviour on removal, meaning whether new tissue is damaged, whether residues remain and whether removal is painful. All three are measurable rather than matters of clinical impression alone.
Types of dressing. Gauze is traditional and inexpensive, assessed for absorption and fibre shedding. Films are transparent and waterproof, assessed for moisture transmission, adhesion and conformability. Foams absorb heavily, assessed for absorption, pressure distribution and moisture transmission. Hydrocolloids maintain moisture, assessed for absorption, gel formation and adhesion. Alginates absorb heavily and gel, assessed for absorption, gelling and ease of removal. Hydrofibres absorb vertically, assessed for absorption, fluid lock and integrity. And dressings with active ingredients, such as antimicrobials, are assessed for release performance and functional verification. Emphasis differs by type, while absorption and moisture transmission are common to most.
Examining absorption. Absorption capacity covers fluid absorbed per unit area. Absorption rate covers how quickly, which affects handling of heavy exudate. Directionality covers whether absorption is vertical, locking fluid within the dressing, or lateral, which may macerate surrounding skin. Retention under compression covers whether fluid returns to the wound surface when pressed, which matters because dressings are compressed in use by body weight, bandages and clothing. And structural retention after absorption covers whether the dressing disintegrates or can still be removed in one piece. The test fluid should simulate exudate rather than being pure water, since exudate contains protein and electrolytes whose effect on absorption differs from water. Retention under compression is a practical discriminator: two dressings may absorb equally while one releases fluid back under pressure and the other holds it, and their clinical behaviour differs markedly.
Examining moisture transmission. Moisture vapour transmission rate covers vapour passing per unit area and time. Its interaction with absorption determines exudate management capability together. Balance matters, since too little transmission causes pooling and maceration and too much dries the wound. Transmission in different states may differ between dry and wet dressings. And test conditions must specify temperature and humidity gradient. Transmission after absorption deserves separate measurement, since some dressings change structure when wet and their transmission changes with it.
Interaction with the wound. Adhesion covers how strongly the dressing adheres to the wound bed; ideally it does not adhere to the wound while adhering moderately to surrounding skin. Removal force covers the force required, which affects pain and tissue damage. Residues cover material left in the wound after removal. Fibre shedding matters for gauze and fibrous dressings, since shed fibres become foreign bodies in the wound. And the effect on new tissue covers damage to granulation and epithelium during removal. Removal force and residues are key to both user experience and clinical outcome: an over-adherent dressing hurts on removal and may damage the wound, and this should be measured rather than described.
Adhesion and surrounding skin. For dressings with adhesive borders, initial adhesion covers how well it sticks. Durability covers retention during wear. Adhesion under moisture covers perspiration, exudate and bathing. Effect on skin covers damage from repeated application and removal. And conformability covers fit over joints and mobile areas. Adhesion under moisture is the practical assessment, since periwound skin is often damp.
Materials and biocompatibility. Contact nature involves breached tissue, which is demanding. Contact duration covers the wear period. Items cover cytotoxicity, irritation and sensitisation as a base. Active ingredients bring additional considerations covering released substances and their local and systemic effects. And processing residues cover what manufacture leaves. Dressings with active ingredients face a wider evaluation, since beyond the materials themselves the released agent must be evaluated and both the quantity and the duration of release verified.
Sterility and packaging. Most dressings are sterile products. Sterilisation must be compatible with materials and active ingredients, some of which are sensitive. Packaging must provide a sterile barrier and permit aseptic presentation. Shelf life covers the stability of materials and function. And in-use considerations cover whether packs are single or multiple.
How we handle it. For dressing testing we suggest using simulated exudate rather than pure water for absorption and moisture transmission, since exudate composition affects absorption behaviour and water-based data diverge from reality. Retention under compression is an item we suggest adding, since it separates absorbing from holding and holding is what matters clinically. For products with active ingredients, verifying release performance is the focus, covering quantity, rate and duration, since those data underpin the functional claim.
