English version

Conclusion. Patient monitors carry a long list of test items spanning electrical safety, electromagnetic compatibility, the performance of each parameter module, the alarm system, and general mechanical and environmental requirements. Looking at where failures actually occur, two areas predominate: the alarm system and parameter measurement accuracy. Alarm systems involve many details, covering alarm conditions, priorities, auditory and visual characteristics, delay, silencing logic and distributed alarms, any of which failing constitutes nonconformity. Parameter accuracy is affected by sensors, signal processing, algorithms and test conditions, and requirements differ between modules. Other items also produce failures, but the concentration is in these two.

Organising the items. Electrical safety covers leakage current, dielectric strength, earthing, temperature rise and single fault conditions. Applied parts cover classification, isolation and defibrillation protection where applicable. Electromagnetic compatibility covers emissions and immunity. Parameter performance covers the accuracy and range of each module. The alarm system covers conditions, priorities, auditory and visual signals, delay and logging. Display covers legibility, refresh and waveform presentation. Data management covers storage, review and export. Networking covers connectivity and distributed alarms. Mechanical and environmental testing covers drop, vibration, temperature, humidity and ingress protection. And battery testing covers endurance, changeover, charging and indication. Organise submission preparation along this structure, confirming documentation and samples category by category.

Examining the alarm system. Alarms are the monitor's core safety function. Alarm conditions cover limits for each parameter and the logic determining an excursion. Priorities cover whether the assignment among alarms is sensible and conforming. Auditory characteristics cover pitch, rhythm, volume and distinction between priorities. Visual characteristics cover colour, flash rate and position. Alarm delay covers the time from condition to signal. Silencing and pausing cover time limits and automatic restoration. Alarm self-test covers whether power-on self-test verifies alarm function. Technical alarms cover equipment status such as sensor or lead detachment and low battery. Alarm logs cover recording of alarm events. And distributed alarms cover reliability and delay when alarms travel over a network. Silencing time limits are a frequent nonconformity: a design permitting indefinite silencing is problematic, since if nobody restores it the alarm function is effectively gone. Technical alarms are also easily overlooked: without an alarm on lead detachment, a flat line may be displayed while the user assumes monitoring is normal.

Examining parameter accuracy. Requirements differ by module, and several points matter. Test conditions require specified simulators or standard signal sources. The measurement range must be verified across the whole claimed span rather than at mid-scale only. Extreme values near the range boundaries need checking. Behaviour under interference covers motion artefact, poor electrode contact and other signal disturbance. Response time covers how quickly the reading follows a change. And applicability across populations covers differences between adult, paediatric and neonatal modes. Behaviour under interference is what matters clinically: accuracy on ideal signals is straightforward, while real clinical signals carry assorted disturbance and behaviour there reflects actual performance.

Defining essential performance. For a monitor this typically includes measurement accuracy within the specified range; effectiveness of the alarm function; correctness of waveform display; and the absence of misleading output. The last matters greatly: equipment ceasing to work is one kind of failure, but displaying an incorrect value that the user takes as real data is a greater risk. Write that into the acceptance criteria for immunity testing.

Common causes of nonconformity. Alarm sound characteristics not conforming in pitch, rhythm or volume. Inappropriate priority assignment, with serious conditions given too low a priority. Faulty silencing logic, whether unlimited silencing or no automatic restoration. Parameter accuracy failing at the edges of the range while passing mid-scale. Parameter drift under immunity testing, with values shifting and no indication. Leakage current exceeding limits, patient leakage especially. Alarm delay exceeding the required response time. And missing technical alarms, with abnormal equipment states unannounced. Parameter drift under immunity is the insidious one: the equipment has not stopped, the display looks normal, and the values are already inaccurate with no way for the user to tell.

Preparing for submission. Provide complete technical documentation covering circuit diagrams, component lists, software versions and technical descriptions of each parameter module. Provide alarm configuration details covering conditions, priorities and configurable ranges. Provide the essential performance definition and acceptance criteria. Prepare the accessories each module needs, including sensors, leads and cuffs. Provide simulator interface details so that signal sources can be connected. Prepare enough samples, since some testing is destructive. And describe the operating modes, covering switching and differences between adult, paediatric and neonatal settings.

How we handle it. For patient monitors we confirm the alarm configuration and the essential performance definition first, since those determine the acceptance criteria for immunity testing. Without clear criteria, value drift observed during testing cannot be judged. On parameter accuracy we suggest measuring at several points near the boundaries of the claimed range, since boundaries are where accuracy most often falls short and mid-scale verification alone is incomplete.
