English version
Conclusion. An anaesthesia machine mixes medical gases in proportion, adds anaesthetic vapour and delivers the mixture to the patient through a breathing system. The higher-risk failure mode in that process is delivering a mixture whose oxygen concentration is too low, causing hypoxia. These machines therefore carry layered measures against hypoxia: oxygen ratio interlocking, low-oxygen alarms, oxygen concentration monitoring, oxygen supply failure protection and emergency oxygen flush. All of these must be measured at submission, because they complement one another and failure of any layer reduces the overall safety margin.
System composition. Gas supply connects to piped supply or cylinders. Flow control adjusts and displays the flow of each gas. Ratio protection prevents the oxygen concentration falling too low. The vaporiser vaporises anaesthetic agent and controls its concentration. The common gas path delivers the mixture. The breathing system conveys gas and absorbs carbon dioxide. The ventilator provides mechanical ventilation. Monitoring covers oxygen concentration, pressure, flow and agent concentration. Alarms indicate abnormalities. And scavenging removes waste anaesthetic gas. Each part carries its own examination items, and the interaction between parts also requires verification.
Examining hypoxia protection. This is the focus of anaesthesia machine testing. Oxygen and nitrous oxide ratio interlocking links the two flows so that adjusting one changes the other and oxygen concentration cannot fall below the limit; verify across the whole flow range. Oxygen concentration monitoring and alarm covers measurement of actual delivered concentration with an alarm below the setting. Oxygen supply failure protection cuts or reduces nitrous oxide when oxygen pressure falls, with an alarm. Emergency oxygen flush delivers pure oxygen rapidly, bypassing flowmeters and vaporiser. And vaporiser output limiting prevents excessive concentration. Verify these by simulating real failure scenarios, deliberately reducing oxygen pressure and setting a low oxygen ratio to see whether the protection operates. Verifying interlocking across the whole flow range matters: some machines interlock correctly at common flows and fail at very low or very high ones.
Examining the vaporiser. Examine output accuracy, meaning the deviation between set and delivered concentration. Examine the effect of flow, measuring output concentration at different carrier gas flows. Examine the effect of temperature, both ambient and vaporiser. Examine the effect of carrier gas composition. Examine the effect of tilting, meaning whether tilting during transport produces abnormal output. Examine interlocking where several vaporisers are fitted, preventing simultaneous operation. And examine filling keying, with agent-specific filling ports. Abnormal output after tilting deserves verification, since tilting occurs during handling and liquid agent entering the bypass can raise output concentration sharply. Multi-vaporiser interlocking is equally critical, since delivering two agents simultaneously can produce an overdose.
Examining the breathing system. Sealing covers leakage throughout. Resistance covers inspiratory and expiratory resistance. Unidirectional valves require verification of opening pressure and reliability, since failure causes rebreathing. Carbon dioxide absorption covers absorbent efficiency and how exhaustion is judged. Compliance covers the compressible volume. And cleaning and disinfection cover the tolerance of reusable parts. Valve failure causes carbon dioxide rebreathing, which may appear on monitoring as a rising inspired carbon dioxide concentration; measure this item directly.
Examining ventilation. Ventilation on an anaesthesia machine resembles a ventilator's, covering parameter accuracy, mode implementation, alarms and behaviour across lung parameters. The distinguishing feature is that ventilation and gas delivery share a circuit, so parameter accuracy is affected by circuit compliance, fresh gas flow and gas composition, all of which must be considered in verification.
Monitoring and alarms. Oxygen concentration monitoring covers accuracy, response time and alarm thresholds. Airway pressure monitoring covers high, low and sustained pressure. Volume monitoring covers tidal and minute volume. Agent concentration monitoring applies where fitted. Carbon dioxide monitoring applies where fitted. And alarm priorities and characteristics follow the same requirements as other medical electrical equipment. Oxygen monitoring response time deserves attention, since it determines how quickly an abnormal concentration is detected.
Examining gas supply. The supply pressure range covers correct operation within specification. Abnormal pressure covers behaviour and alarms when pressure is too high or too low. Supply changeover covers switching between piped supply and cylinders. Misconnection prevention requires keyed connectors for different gases, since misconnection has serious consequences. And leakage requires checking the supply pipework. Misconnection prevention is a basic requirement: reversing oxygen and nitrous oxide would be catastrophic, and the mechanical keying must be verified as effective.
Preparing for submission. Provide a complete gas path diagram to establish the examination points. Provide component specifications for vaporisers, flowmeters and valves. Provide the matching breathing systems claimed as compatible. Provide gas connection details and the gases used. Describe the ventilation modes. Provide alarm configuration details. And prepare sufficient anaesthetic agent and gas, which vaporiser testing consumes.
How we handle it. For anaesthesia machines we measure each layer of hypoxia protection individually, covering ratio interlocking, oxygen concentration alarms, oxygen failure protection and emergency flush, verifying at several flow points rather than at common flows alone. On vaporisers we suggest always including output after tilting and multi-vaporiser interlocking, both corresponding to situations that occur in use and that routine testing readily skips.
