English version
Conclusion. Transfusion and infusion devices, including infusion sets, transfusion sets, extension lines, stopcocks and indwelling catheters, are high-volume single-use products. They contact medicinal solutions or blood directly and establish a pathway from outside the body into the vasculature. Three concerns follow. Particulates, since particles released by the device enter the bloodstream and may cause embolism or tissue reaction. Chemical residues, since substances migrating from materials into the solution enter the body with it. And connection reliability, since disconnection causes blood loss, air entry or leakage of solution. The remaining items covering physical performance, drip rate, filtration, biocompatibility and sterility address function and safety.
The item list. Physical performance covers tensile strength, connection strength and pressure resistance. Leakage covers positive and negative pressure. Drip rate covers drop factor and flow control. Filtration covers filter effectiveness. Particulates cover counts in rinse fluid. Chemical performance covers reducing substances, metal ions, acidity and alkalinity, evaporation residue and ultraviolet absorbance. Biocompatibility covers blood contact items. Sterility and pyrogens cover sterility and endotoxin. Packaging covers barrier, integrity and shelf life. And marking covers product information and warnings.
Examining key components. The spike requires penetration force, sealing after penetration and compatibility with container closures. The drip chamber requires volume, transparency, ease of observation and prevention of air entry. Tubing requires flexibility, kink resistance, transparency and pressure resistance. The flow regulator requires adjustment range, accuracy and stability, and resistance to slipping during prolonged use. Filters require filtration effectiveness, behaviour when blocked and gas venting. Injection sites require sealing after penetration and tolerance of repeated puncture. And connectors require connection strength, freedom from leakage and compatibility with other devices. Flow regulator slippage is a real problem: a drip rate set correctly may change during prolonged infusion as tubing creeps or the regulator slips, and long-duration stability verification is warranted.
Examining particulates. Sources include the materials themselves, processing residues, cut surfaces and packaging. Measurement rinses with a specified fluid and counts particles by number and size distribution. Limits apply by size band. Influencing factors include materials, processing, cleaning, packaging and sterilisation. And particulates in actual use include closure fragments generated by spiking. Closure fragments are a real particulate source: spiking an infusion bottle's rubber closure can shear off fragments, and spike design, particularly tip geometry, affects how many are produced, which warrants verification.
Examining chemical requirements. These products carry a group of chemical items. Reducing substances reflect the quantity of oxidisable material. Metal ions cover heavy metals particularly. Acidity and alkalinity are expressed as the difference between extract and blank. Evaporation residue reflects non-volatile matter in the extract. Ultraviolet absorbance reflects organic content. And specific substances such as plasticisers apply where plasticised materials are used. Plasticiser migration deserves attention in products using plasticised materials, since certain solutions, particularly lipid-containing ones, increase plasticiser extraction.
Examining physical performance. Connection strength covers the force to separate each joint. Tubing kink resistance covers whether bending occludes the lumen. Pressure resistance covers freedom from leakage and rupture at specified pressure. Negative pressure sealing applies where suction is involved. Transparency allows solution and air to be seen. And drip chamber flexibility allows squeezing to prime. Connection strength is usually weakest where different materials meet, such as rigid connector to flexible tubing and metal needle to plastic hub.
Sterility and pyrogens. Sterility requires sterilisation validation and sterility testing. Endotoxin requires limits on pyrogenic substances. Sterilisation compatibility with materials must be confirmed. And the packaging sterile barrier requires integrity and shelf life. Endotoxin is a critical item for infusion products, since it enters the bloodstream directly and can cause pyrogenic reactions while surviving routine sterilisation.
Preparing for submission. Provide the complete product composition covering each component's material. Describe the solutions the product suits, which informs compatibility assessment. Provide the products used in combination, covering which containers and needles. Prepare sufficient samples, since chemical and particulate items consume a good number. Describe the sterilisation method and parameters. And provide packaging samples for integrity verification.
How we handle it. For these products we suggest particulate testing under rinse conditions close to reality, and attend to fragments generated by actual spiking, since those particulates do not appear in routine rinse testing while entering the fluid path in real use. On flow regulation we suggest prolonged drip rate stability verification, since real infusions may run for hours while regulator slippage and tubing creep develop gradually.
