English version
Conclusion. Orthopaedic implants, whether bone plates, intramedullary nails, joint prostheses or spinal fixation, share the characteristic of being implanted long-term and carrying load. Two core examinations follow. Fatigue, because loading is cyclic through walking and activity and an implant must withstand millions of cycles over years or decades; fatigue is the principal failure mechanism for these products. And wear, because for products with relative motion such as joint prostheses, wear particles can provoke tissue reactions affecting long-term outcome. The remaining items, covering static strength, materials, surfaces, sterility and biocompatibility, arrange themselves around these two.
Organising the items. Static mechanics covers strength, stiffness and torsion. Fatigue covers life under cyclic loading. Wear covers the quantity and particles generated by the bearing couple. Dimensions and fit cover dimensional accuracy and component fit. Materials cover composition, microstructure and mechanical properties. Surfaces cover topography, roughness and coatings. Corrosion covers behaviour in simulated body fluid. Biocompatibility follows long-term tissue contact. Sterility and packaging cover sterilisation, barrier and shelf life. And instrumentation covers implantation tools and their use.
Designing fatigue testing. Loading should simulate the actual direction and point of application, which differ between implant sites. Load magnitude follows expected physiological loading, allowing for body weight, activity level and possible peak loads. Cycle count converts from expected service years and activity frequency. Environment should be simulated body fluid, since corrosion combined with fatigue shortens life. Temperature should be body temperature. Test configuration should simulate the actual fixation and support, since different fixation produces entirely different stress distributions. And failure criteria cover fracture, loosening and excessive permanent deformation. Test configuration is where designs most readily go astray: differences between how the implant is fixed and loaded in the body and in the test fixture make results diverge from reality. Design the configuration to approach the actual anatomical and fixation conditions.
Designing wear testing. For joint products, the motion should simulate the actual joint's path, with multi-axis motion closer to reality than single-axis. The load profile should simulate variation through the gait cycle. Cycle count converts from expected service years. The lubricant should simulate joint fluid. Wear should be measured gravimetrically or geometrically. And wear particles should be analysed for number, size and morphology, which influence tissue reaction. Particle characteristics say more about biological effect than total wear volume, since size and morphology determine the degree of tissue response.
Examining materials. Composition must conform to the material standard. Microstructure covers grain size, phase composition and defects. Mechanical properties cover strength, elongation, hardness and fracture toughness. Impurities and inclusions matter as potential fatigue origins. Processing effects cover the influence of cold work and heat treatment. And welds and joints, where present, require property verification. Inclusions are a common origin of fatigue failure, and material cleanliness markedly affects fatigue life, making it a key parameter for high-performance implant materials.
Examining surfaces. Surface roughness affects friction, wear and bone attachment. Surface treatment covers the results of anodising, blasting and etching. Porous coatings for bone ingrowth require porosity, pore size and bond strength. Bioactive coatings, where present, require composition, thickness, bond strength and degradation behaviour. And surface cleanliness covers processing and treatment residues. Coating bond strength is critical for coated implants, since delamination generates particles and impairs bone integration; measure it in both shear and tension.
Corrosion and ion release. Static corrosion covers behaviour in simulated body fluid. Fretting corrosion covers the combined action of micromotion and corrosion at component interfaces, which is the concern for modular implants. Galvanic corrosion covers electrochemical action between dissimilar materials. And ion release covers metal ion quantities released during long-term implantation. Fretting corrosion needs specific examination in modular designs, since taper connections experience micromotion, and once micromotion disrupts the passive film corrosion accelerates and may release substantial particles and ions.
Instrumentation. Implants require matching instruments. Instrument strength covers loading during implantation. Fit with the implant covers reliability of gripping, guiding and positioning. Instrument durability covers repeated use and reprocessing. Reprocessing tolerance matters since instruments are reusable. And misuse protection prevents using the wrong instrument or assembling in the wrong orientation. Instrument problems affect implantation quality directly, so their verification should not be treated as secondary.
How we handle it. For orthopaedic implants we agree the fatigue test configuration with the client, simulating the actual fixation and loading as closely as possible, since configuration differences change results significantly. On environment we suggest testing in simulated body fluid at body temperature, since dry ambient data read optimistically while actual implantation carries load in a fluid environment over years. For modular products we suggest adding fretting corrosion examination, which addresses the real risk at component connections.
