English version
Conclusion. Physiotherapy equipment delivers energy into the body to achieve a therapeutic effect, whether electrical current, ultrasound, light, magnetic field or heat. Energy output is both its function and its source of risk. Two questions are central. Is the output accurate, meaning the deviation between set dose and actual delivery; too little fails to achieve the effect and too much may cause injury. And can it exceed limits, meaning whether output could exceed safe values under normal or fault conditions. Those two determine the testing focus; the remaining items covering electrical safety, electromagnetic compatibility and mechanical and environmental testing follow the general requirements.
Measuring each energy type. Low frequency current requires measurement of current, voltage, waveform, frequency and pulse width by electrical parameter measurement. Medium and high frequency current requires power, frequency and modulation by power and waveform measurement. Ultrasound requires acoustic power, intensity and frequency by acoustic power measurement. Light, whether infrared, laser or light-emitting diode, requires irradiance, wavelength and energy by optical radiation measurement. Magnetic fields require flux density and frequency by field measurement. Conducted heat requires temperature and heating rate by temperature measurement. And shock waves require pressure and energy density by pressure measurement. Methods and equipment differ by energy type, so confirm before submission that the laboratory holds the corresponding measurement capability.
Examining output accuracy. Verify across the whole output range rather than at mid-scale, covering both low and high settings. Examine output under different loads, since body impedance varies between people and output should remain stable across the range. Examine stability during prolonged output for drift. Examine parameter interaction, checking whether changing one alters others. And examine agreement between displayed and measured output. Behaviour under different loads is critical in use: electrical stimulation output is affected by electrode contact impedance, and poor contact may reduce output, while some devices raise voltage to maintain output, at which point local current density may rise and burn.
Examining output limits. Maximum output covers the ceiling under any setting or operation. Output under single fault covers whether output could exceed limits when a control component fails, which is the critical item. Unintended output covers spikes at switch-on, switch-off, parameter adjustment and mode change. Cumulative dose limits cover the total energy in one treatment. And time limits cover maximum treatment duration and automatic stopping. Output under single fault must be measured: deliberately disable the control element and see whether hardware measures limit the output. In a device where output is controlled by software alone, software failure can leave output uncontrolled.
Examining applicator parts. Output reaches the body through applicators. Electrodes require examination of contact area, impedance and current distribution uniformity, since uneven distribution produces high local current density and burns. Ultrasound transducers require acoustic field distribution, effective radiating area and beam non-uniformity ratio. Light output windows require spot distribution and edge effects. Heating surfaces require temperature distribution uniformity. Applicator temperature rise covers the contact surface after prolonged use. Biocompatibility covers skin-contacting materials. And durability and cleaning cover tolerance of repeated use and disinfection. Current distribution uniformity is critical for stimulation devices: current density at electrode edges normally exceeds that at the centre, and excessive non-uniformity can burn the edges. Measure current density distribution rather than total current alone.
Examining safety functions. Output interruption should stop delivery and alarm when an electrode detaches or contact is lost. Emergency stop must be reachable by patient and operator. Gradual ramping should raise output progressively rather than applying it suddenly. Parameter confirmation should require acknowledgement before output, preventing mis-setting. And treatment timing should stop automatically at the end. Electrode detachment detection is a necessary function for stimulation devices: partial detachment reduces contact area and raises current density, and if the device continues delivering at the set value the risk of burn arises.
Verifying treatment parameters. Parameter ranges cover what can be set. Parameter resolution covers the smallest adjustment step. Preset programmes, where provided, require verification of their parameters. And parameter retention covers behaviour after power loss. Verify preset programme parameters individually, since users may apply a preset without setting parameters themselves, and an incorrect preset produces incorrect treatment parameters directly.
Preparing for submission. Provide design values for output parameters for comparison. Describe each treatment mode and its parameter ranges. Provide the accompanying applicators including electrodes, transducers and light guides. State the applicable load range, meaning assumptions about body or acoustic impedance. Provide a list of preset programmes for verification. And prepare sufficient consumables such as single-use electrodes.
How we handle it. For physiotherapy equipment we treat output limiting under single fault conditions as a priority measured item, deliberately disabling the control element and verifying that hardware measures limit the output. That item addresses the risk the patient faces when control fails. We also suggest measuring energy distribution at the applicator, covering current density, acoustic field or spot distribution, since acceptable total output does not mean local values are within limits, and local excess is exactly what causes burns.
