English version
Conclusion. Testing laser therapy equipment centres on two things. Hazard classification, since the laser's output level determines its hazard class and the class determines what protective measures, markings and conditions of use are required; misclassify and the protective requirements follow the error. And output parameters, since wavelength, power, energy and pulse duration determine both therapeutic effect and the action on tissue, so deviation between setting and actual output affects treatment and safety directly. The remaining items covering electrical safety, electromagnetic compatibility and mechanical and environmental testing follow the general requirements, with laser-specific additions.
Basis for classification. Classification rests on several factors. Wavelength, since tissue absorption and hazard mechanisms differ and ocular hazard in particular is wavelength-dependent. Output power or energy, with continuous wave assessed by power and pulsed operation by single pulse energy and average power. Pulse characteristics of duration and repetition rate. Beam divergence, which determines irradiance at different distances. And accessible emission, meaning how much radiation can be measured where contact is possible. The classification determines marking requirements, protective requirements, conditions of use including whether a dedicated area is needed, operator qualification requirements, and requirements for interlocks and other safety devices. The measurement position for accessible emission is critical: the distance and aperture at which measurement is made affect the classification directly, and those conditions are explicitly specified.
Measuring output parameters. Wavelength covers deviation of actual output from nominal. Power or energy covers measured values at each setting. Stability covers drift during prolonged output. Pulse parameters cover duration, repetition rate and single pulse energy. Beam characteristics cover spot size, energy distribution and divergence. Switching behaviour covers output at start and stop. And parameters at the treatment distance cover actual irradiance at different working distances. Energy distribution within the spot affects treatment uniformity: uneven distribution means centre and edge act with different intensity, which can produce uneven treatment or local excess.
Examining the aiming beam. Most therapeutic lasers are invisible and need a visible aiming beam to indicate position. Coaxiality covers agreement between aiming and treatment beam positions, since deviation means treating somewhere other than where the operator sees. Aiming beam brightness covers visibility under treatment room lighting. Adjustability covers whether brightness can be varied. And aiming beam reliability covers whether treatment output should be inhibited if the aiming beam fails. Inhibiting output on aiming beam failure is a sound safety design, since without a visible aiming beam the treatment position cannot be confirmed.
Verifying protective measures. Key control prevents unauthorised use. Interlocks on doors and guards stop output when opened. Emergency stop cuts output quickly. Distinguishing standby from emission requires clear state indication, preventing an emitting device being taken for one on standby. Emission indication requires clear audible or visual indication during output. Delayed start gives the operator reaction time between arming and emission. Inadvertent operation protection covers footswitches and handpieces. And aperture protection covers shuttering when not in use. State indication matters greatly: an operator believing the device is on standby while it is actually armed faces serious consequences from inadvertent triggering, so indication must be unambiguous and hard to misread.
Protective eyewear. Eyewear specification must match the device's wavelength with an appropriate protection level. Supply should either include suitable eyewear with the device or specify the requirement explicitly. Eyewear marking should state wavelength and protection level. Visible light transmittance should not impede normal operating vision. And multi-wavelength devices require eyewear covering every wavelength. Eyewear specification must be marked explicitly, because mismatched eyewear provides no protection and operators cannot judge from appearance.
Environmental requirements. Controlled areas are required for higher-class lasers. Warning signage is required at area entrances. Reflective surface control prevents unintended exposure. Combustible material control matters because lasers can ignite certain materials, particularly in oxygen-enriched environments. And plume management addresses the smoke produced when laser energy acts on tissue, which requires extraction. Fire risk in oxygen-enriched environments is a real surgical hazard: high oxygen concentration makes ignition of drapes and other combustibles easier, and the instructions should warn accordingly.
Preparing for submission. Provide laser parameters covering wavelength, power and pulse characteristics. Provide optical system information covering beam delivery and focusing. Describe treatment modes and parameter ranges. Provide the delivery components including fibres, handpieces and output heads. Provide the matching protective eyewear. And describe the use scenario, which informs classification and environmental requirements.
How we handle it. For laser equipment we treat measured output at each setting and aiming beam coaxiality as priority items, the first bearing on dose accuracy and the second on treatment position accuracy. On safety devices we measure the operation of interlocks, emergency stop and state indication rather than reviewing design documentation, since these are the direct measures preventing unintended exposure.
